Pillars of Eternity sijoittuu fantasiamaailmaan nimeltä Eora, jossa mm. muskettisoturit taistelevat pistoolien ja tykkien avustuksella. Pelissä on niin sanotut ikuisuuden pilarit, jotka ovat salaperäisiä jalokivimuodostelmia, joita kutsutaan nimellä adra. Jatko-osan tarina jatkuu suoraan siitä, mihin edellisessä osassa päädyttiin. Tällä kertaa seikkailuun lähdetään laivalla ja mukana seilaavat muutamat tutut hahmot, kuten Edér ja Aloth. Hahmoluokkia on yhteensä 11, jotka ovat barbarian, chanter, cipher, druid, fighter, monk, paladin, priest, ranger, rogue ja wizard. Jokaisella hahmoluokalla on lisäksi vähintään kolme eri alaluokkaa, jotka tarjoavat erilaisia vahvuuksia ja heikkouksia. Pelissä on viisi eri vaikeustasoa, joten kaikentasoiset pelaajat pääsevät nauttimaan pelistä.
